Ripple™ is a personalised health coaching programme developed to help you better manage your long-term conditions through the building of positive habits, brought to you by Sheares Healthcare in partnership with Minmed.

 

We understand the importance of providing you with the best support possible, and to do that we need to gather some personal information from you.

 

By clicking 'I agree',  you're agreeing to share your name and contact details with the Ripple™ programme team, which will be used for the purposes of the introductory session and to register you into the Ripple™ programme, if you elect to proceed. You're also giving Sheares Healthcare, our affiliates, and their third-party service providers permission to collect, use, disclose, and process your personal data for the purposes of the introductory session and the programme. Additionally, you're allowing the Ripple™ programme team to collect your medical records from, and share information about your participation in the programme with your referring doctor/ clinic, or other medical professional whose contact details you provide to Sheares Healthcare for the purpose of this programme.
